Elevate Neuro Rehab can work with both self-managed and plan-managed NDIS participants.

If you are in the process of applying for access to the NDIS and require an Occupational Therapist assessment for supporting documentation, we are more than happy to help.

Support Coordinators

We highly value the partnership we establish with Support Coordinators, and we appreciate your consideration in choosing us to provide services for your clients. Our streamlined processes and commitment to delivering top-notch care allow us to typically accommodate new clients within 1-3 weeks for both Occupational Therapy (OT).

As a close-knit team of exceptional clinicians, we intentionally maintain a small caseload. This approach ensures that we can uphold excellent standards of service and dedicate ample time and attention to each client’s needs.

Functional Capacity Assessments

We are pleased to offer Functional Capacity Assessments (FCAs) as part of our comprehensive range of services. Although our primary focus is on delivering the therapy recommended by these assessments, we understand that there are situations where an FCA becomes essential to ensure your client receives the appropriate support. Rest assured, we are more than willing to assist in such
cases. Our reports are meticulously crafted to provide you with the following benefits:

Holistic, Expert Evaluation: Our assessments are conducted by seasoned clinicians with over a decade of experience in the occupational therapy field. Their expertise ensures a thorough and professional evaluation of your client’s functional capacity.
Utilization of Proven Tools: To gather accurate and insightful data, we employ recognized tools such as the Care and Needs Scale, WHODAS, WHOQOL, CIQ (Community Integration Questionnaire), and PEDICAT (Paediatric Evaluation of Disability – Computer Adapted Version). These tools contribute to a comprehensive understanding of your client’s unique requirements.
